It sounds like there is rocks bouncing around in my cat, does that mean its plugged or needs replacing? What would happen if I just cut the cat out and left an open down pipe? would it be overly noisy? would I have exhaust coming in to the van? just want to know if this would be ok or should I forget it and have a muffler shop fix it up.

Yup, it's toast. New ones are not too bad a price these days. Probably best to go for a new high flow one, emissions testing might not be too far off in your part of the world.
