Hack the stock flange off, weld a new flange on. Whatever bolt pattern you want.
Or, like you said, build a custom manifold. Totally possible, but people will piss and whine about how your design sucks and how you'd get N horsepower if you would have done it differantly.. Blah blah. If it works, it works..
Also, you can use the turbonetics 0.63 turbine housing. They still make it, i believe, and it's still availible from a few TD vendors. It has the right pattern on it.
If it was me, i'd just stick to the stock T3 for a while. People have run into the 12's with it, so it's not exactly a bad turbo. If you have a mitsu right now, it'd be a great upgrade.

I hacked my existing studs off, milled the entire flange, drilled, tapped and installed new studs to the regular T3 flange pattern.

no pics no.
Problems, well if i did it myself i would of had problems. I cut the studs off, machine shop milled the flange and drilled/tapped holes. Make sure a good machine shop does this so the holes are nice and straight and square. If there off at all it will give you headaches for years to come.
Picture in your head what the flange would look like with this done to it...thats exactly what it looks like.

I made a plate out of 1/2 plate, bolted it to the manifold and ground down the lower portion and also the manifold, essentially lowering the turbo 1/2 or so. Then drilled 2 new lower holes, one upper hole and used one original stud on the turbo and one to hold the plate down. So far, 22000 kms and still works awesome!
