Codes 12, 15, 54 What Gives?

I'm trying to get my car inspected and have codes 12,15,54 and 55(end).
What do they mean and how do I address them. Car runs rich per inspector.

12 Battery or power line recently disconnected

15 is No speed/distance sensor signal
( Get a new S/D sensor )

54 is No fuel injector sync pickup signal during engine rotation
(Probably the HEP)

UPDATE

Replaced the hall effects and codes went bye bye. Passed with ease. If your car runs rich and a new o2 does nothing try the hall efects or pick-up assembly as its also known by.
